Level 1 — Early Concern

First Faces Referral & Connection Pathway

Notice → Listen → Connect → Support → Follow Up. The first adult does not need to solve the problem — the first adult makes sure the concern reaches the right support. Use this form to document the handoff and close the loop.

Referral details

1. NOTICE

Student returned from an absence, arrives late repeatedly, shows a change in pattern, or shared a concern.

What was noticed?

2. WELCOME + LISTEN

Reconnect first. If appropriate, speak privately. Use curiosity rather than assumptions: “Is everything okay?” “Is there anything making it hard to get here that we may be able to help with?”

Was a barrier or support need identified?

3. CONNECT

Share the concern through the site's established attendance/student-support pathway. Suggested handoff: “Thank you for telling me. I'm going to connect you with someone who may be able to help.”

Example need / barrier

4. SUPPORT + OWNERSHIP

The receiving team/person identifies: What support is needed? Who owns the next step? When will it happen? The student should not be sent from adult to adult to repeatedly retell the story.

5. CLOSE THE LOOP

Did the connection happen? Did the student/family access the support? Did attendance improve? Continue, adjust, or move to the next level of support based on evidence.

Did the connection happen?
Based on evidence, we will…

IMMEDIATE SAFETY / PROTECTION CONCERN: Do not use the routine attendance pathway first. Follow school safety, emergency, and mandated-reporting procedures immediately.
